Client Agreement

  • I understand that life can be unpredictable, I regret that late arrivals may not receive extensions of scheduled appointments. In special circumstances, when my schedule will allow I may be able to accommodate a partial or full appointment.

    This will be at my discretion and only with proper advanced notice of your late arrival. If late arrival is inevitable, your service may be shortened to keep on schedule and the original treatment time will be charged.

  • All services are performed by Hannah Ash, LMT Ohio and held to the highest standards of the American Massage Therapy Association.

    Massage Therapy is a profession in which the practitioner applies manual techniques, and may apply adjunctive therapies, with the intention of positively affecting the health and well-being of the client.

    I do not diagnose or prescribe for medical conditions nor am I allowed to provide treatment for a specific condition without a doctor’s supervision.

    I am required to refer you for diagnosis and to follow the recommendations of your physician.

  • All new clients are required to provide a credit card upon booking. This can be done quickly and easily through the online booking system.

    Upon checkout, guests may choose their method of accepted payment and are not obligated to use the card on file for payment.

    I hold this information as part of the cancellation policies and only make necessary changes to your account on the date of your scheduled service.

    Unfortunately, at this time I do not accept insurance or checks.

  • Your therapy times are reserved exclusively for you. Please provide at least a 24-hour notice if you need to reschedule or cancel your treatment.

    I value your business and ask that you respect the scheduling policies. Any cancellations with less than 24 hours of notice are subject to a cancellation fee of 100% of the scheduled service.

    No call No Show

    Clients who miss their appointments without prior notification will be charged in full for the scheduled service. When you miss your appointment with me, I lose your business and the potential business of other clients who could have scheduled an appointment at the same time.

  • Prior to each massage session, the treatment plan will be discussed with you. When scheduling your first visit you will receive a link to the policies and will be asked to agree the consent stating that you have read the information, understand it, and agree to comply with the professional policies and procedures/client agreement.

    By scheduling with Sōl Deep Wellness you agree that you are receiving this wellness service voluntarily and at your own risk. You agree to report all health conditions that you are aware of and will inform your practitioner of any changes in my health. It is your choice to receive massage therapy, and you give my consent to receive treatment. To the best of your knowledge, the above information is complete and accurate. You agree to release and hold harmless the practitioners of Sōl Deep Wellness, and it's officers and agents, from any claims related to this wellness service, including claims of negligence.

    Payment is due at time of service unless otherwise arranged with your practitioner.

  • Massage is strictly non-sexual, and Hannah has a zero-tolerance policy for sexual harassment of any kind, including but not limited to solicitations for extras, suggestive comments, or inappropriate touch.

    You will be escorted off the property and authorities will be called.

  • In this partnership, it is important that the client is honest and open with the massage therapist.

    Feedback about the effectiveness of the work, situations that may have been uncomfortable for you, massage techniques that you enjoyed, massage techniques that you did not like, etc., is very important.

    Please feel free to share this information with me, as you become aware of it.

    If at any time, you feel that another therapist would better serve you, please ask for a referral.  If I do anything that makes you feel uncomfortable, tell me immediately. I will not be offended as I hold my massage therapy policies to a very high standard.

    Hannah (your therapist) is happy to adjust pressure, temperature, musical volume, work longer on an area or move on if you request it.

    The client may choose to: leave on as much clothing as needed for comfort, refuse any massage methods, stop massage at any time and is free to leave; the therapy door is never locked.

    The client will always be modestly draped. Only the area being massaged will be undraped. The clients will be kept informed of the area to be massaged.

    The breast and genital area will not be massaged under any circumstances. Also, the low back, hip & gluteal area will be massaged only with permission and can be worked through the draping if requested.

  • The discussion between the massage therapist and the client is confidential. The client may or may not choose to talk during the massage. I am happy to listen to your conversation and share my professional expertise. I prefer to not discuss topics of a political, private or sexual nature.

    For clients undergoing chemo and radiation please note that I require a doctor’s note that states the doctor is aware of and agrees to the desired treatment.

  • It is the responsibility of the client to keep the massage therapist informed of any medical treatment currently being taken, and to provide written permission from the physician, chiropractor, physical therapist, etc., that the massage may be continued. The client must also keep the massage therapist informed of any changes in health conditions.

    For clients undergoing chemo and radiation please note that I require a doctor’s note that states the doctor is aware of and agrees to the desired treatment.

  • All equipment and linens that come in contact with the client will be cleansed in an approved method. The massage room is disinfected between clients and sanitized each evening.

    Before and after each massage, the massage therapist’s hands, forearms, and feet will be washed with hot soapy water and a disinfecting solution. Any breaks in the skin of the massage therapist will be covered with protective coverings.

    The client will come to the massage therapy appointment clean. The client will inform the massage therapist of any breaks in the skin, and these areas will be avoided.​

    Should the massage therapist or the client contract a contagious disease that could be spread during the massage session, such as a cold, flu, or other droplet or airborne infection, each will inform the other. The decision to reschedule will be handled on a case-by-case manner.
